Forms/Paperwork
Before submitting your paperwork, double check the date of the course, the course
code, component parts and grades (P/F/A). Don't forget to sign the course record
by typing your name in the appropriate section.
Submit the completed paperwork by sending as email attachments to instructor@indyredcrosstraining.org. Please indicate where certificates
should be sent.
Reserve Equipment and Purchase Supplies
